At the beginning of each school year, Hubbard Schools will require parents / guardians to complete online Student Demographic and EMA (emergency medical authorization) forms. OneView is where you will find the district’s annual student forms that previously would have been turned in on paper the first week of school.
The information contained in the online forms helps ensure that we have the most up to date information pertaining to your child. In addition, should any information pertaining to your child change throughout the school year, we ask that you update the information using the OneView system.
The electronic, online forms (Student Health Form, Emergency Contacts, etc) are available
immediately. It would be very helpful if families could complete the online forms this week.
However, we would like all forms completed and submitted by August 31st
