Hubbard Elementary School Kindergarten Registration for the 2021-2022 school year will be held on Tuesday, Thursday and Friday, April 6th, 8th and 9th by APPOINTMENT ONLY. Please call 330-534-1921 ext. 4001 beginning March 1st to make an appointment.
Parents/guardians will come to the elementary school WITH THEIR CHILD at their scheduled time. Upon arrival, the children will attend a brief screening. Due to Covid guidelines, we ask that only ONE parent and the child attend the appointment. Masks and temperature screenings will be required for the parent and child.
Parents/guardians PLEASE MAKE A COPY of the items listed below for the school to keep. Final registration will NOT occur unless ALL COPIES ARE PROVIDED.
Child’s birth certificate (Must be 5 years of age before September 30, 2021)
Immunization record
Proof of residency which includes ONE of the following:
Mortgage/Bank Payment Book with name and addresses
Lease AND current utility bill with resident’s name and address
Most current gas, phone, electric bills with name and address AND two pieces of business mail in a stamped envelope.
Municipal Income Tax Registration
Custody or guardianship records (if applicable). Documentation is needed in those cases dealing with divorced parents or a court ordered guardianship.
Photo ID of parent/guardian for identification purposes: One of three examples below is required:
Ohio Driver’s License
Ohio I.D. card
Other valid type of photo I.D.
