Kindergarten registration for 2019-2020 school year scheduled for March 5-7

KINDERGARTEN REGISTRATION FOR THE 2019-2020 SCHOOL YEAR


Hubbard Elementary School Kindergarten Registration for the 2019-2020 school year will be held Tuesday, March 5, 2019 through Thursday, March 7, 2019.


Parents/guardians should arrive at the elementary school WITH THEIR CHILD between 9 a.m. and 11:30 a.m. OR between 12:30 p.m. and 2 p.m. on Tuesday (March 5), Wednesday (March 6) or Thursday (March 7).


Upon arrival, parents will complete the online registration while their children attend a brief screening. Laptops will be provided and assistance will be available. Due to the number of parents/guardians that may be enrolling prospective kindergarten students, please know you may be expected to wait. We appreciate your patience.


Parents/guardians, PLEASE BRING A COPY OF THE ITEMS LISTED BELOW. Final registration will NOT occur unless ALL DOCUMENTS ARE OBTAINED.


1. Child’s birth certificate (Must be 5 on or before September 30, 2019)

2. Immunization records

3. Proof of residency which includes ONE of the following:

  • Mortgage/Bank Payment Book with name and addresses

  • Lease AND water bill with resident’s name and address

  • Most current UNPAID gas, phone, electric bills with name and address AND two pieces of business mail in the stamped envelope.

  • Municipal Income Tax Registration

4. Custody or guardianship records (if applicable). Documentation is needed in those cases dealing with divorced parents or a court ordered guardianship.

5. Photo ID of parent/guardian for identification purposes: One of three examples below is required:

  • Ohio Driver’s License

  • Ohio I.D. card

  • Other valid type of photo I.D.
