Open enrollment application window to resume May 18

This applies to any non-resident student who is not already enrolled in Hubbard Schools and wishes to apply for open enrollment for the 2020-2021 school year. Application Dates: The window to pick up and return applications is May 18th - May 29th. (Excluding Memorial Day Holiday) Time: Door will be open 9:00 a.m. – 3:00 p.m. Location: Hubbard Administration Office; 108 Orchard Avenue, Hubbard Special Instructions: To adhere to ODH guidelines, applications will be handled through the rear door of the building. Only one family at a time will be permitted to enter the building. While waiting outside, please remember to respect the ODH recommendation of maintaining a 6 foot distance between families. We are not accepting applications for 8th grade due to class size. Hubbard does not accept applications for 12th grade. When returning your application packet, please have COPIES of all required documents. Copies cannot be made for you. The following documents are needed. 1. Completed and Signed application form 2. A copy of the child's birth certificate. If the seal is not seen on the copy, bring the original for verification. 3. A copy of the child's immunization record. 4. A copy of parent/guardian identification - valid driver's license or state ID 5. A copy of child custody documents, if applicable. 6. A copy of documents proving residency - i.e. lease or mortgage statement with most current utility bills (other options are discussed as needed) 7. A copy of the child's most recent report card for students applying for grades 1-11. 8. A copy of high school transcripts for students applying for grades 10 or 11. 9. A copy of additional paperwork if the student has any of the following as part of their education: an IEP with ETR (both documents); 504; WEP; LEP/ELL courses.
